IA app name cookie write failed Error When Creating A macOS USB Installer

If you’re trying to create a macOS USB installer using Terminal and run into the error message … IA app name cookie write failed … the issue may be due to Terminal not having Full Disk Access.

To check, do the following:

  • Go to the Apple menu

  • Select System Preferences

  • In System Preferences, select Security & Privacy

  • In Security & Privacy, go to the Privacy tab

  • In the Privacy tab, scroll down to Full Disk Access

  • Check to see if Terminal is listed under Full Disk Access

If Terminal is NOT listed under Full Disk Access, you will need to add Terminal to give it the necessary permissions.

To set up Terminal with Full Disk Access, do the following:

  • If the Security & Privacy window is locked (the lock at the bottom left corner of the Security & Privacy window will be in the locked position), click the lock and enter your administrator account credentials to unlock the Security & Privacy window to make changes.

  • Once the Security & Privacy window is unlocked (the lock will change to the unlocked position), click the “+” sign then search for and add Terminal. Terminal is in Macintosh HD | Applications | Utilities.

  • Once Terminal is added to Full Disk Access, you can click the lock to secure the Security & Privacy window then close the window.

  • If you have an existing Terminal session open, close out of Terminal completely.

  • Relaunch Terminal and try creating your macOS USB installer again.
